This policy is effective from 25 May 2018


What information do I hold?

Leanne Taghizadeh Counselling retains the following information – 

Name and contact details, including email address, home address and telephone number.  Also, GP and next of kin contact details, in the event of any risk or emergency situations

How I use your information

To provide you with counselling or supervision services
To be able to communicate with you and provide information on above
To arrange, cancel or reschedule any appointments

Who might I share your information with?

In exceptional circumstances, eg., if I feel that you or someone else may come to harm, I will break confidentiality and share information with the relevant authorities.  I would always endeavour to discuss this with you in advance, however, this may not always be possible

Your rights

Under General Data Protection Regulations you have the following rights regarding the data that I collect and process in the delivery of our services. These are:

The right of access to your data, specifically:

The right to confirmation that your data is being processed
The right to access your personal data held by Leanne Taghizadeh Counselling
The right to rectification of your data if it is inaccurate or incomplete
The right to be forgotten – your right to request deletion or removal of data held by Leanne Taghizadeh Counselling
The right to restrict processing of your data by Leanne Taghizadeh Counselling: You have the right to object to me processing your personal information if I am not entitled to use it anymore, to have your information deleted if I am keeping it too long or have its processing restricted in certain circumstances

How long do I keep your information for?

I will keep your personal data in order to provide you with counselling or supervision. Your personal information will be retained in accordance with law and regulation, and I will never retain your data for longer than permitted under law and regulation.
I will keep counselling records for a maximum of 7 years, in accordance with legal and indemnity insurance requirements


Security

In order to prevent unauthorised access or disclosure of your data, I have put in place suitable physical and electronic procedures to safeguard and secure the information I collect
For example, all data is stored securely on designated drives and servers and is only accessible to authorised individuals
